I understand that there are risks involved with sports, including the sport of cricket. I understand the nature of this sport and have been given an opportunity to ask questions about it. I understand that ‘The Institute Of Cricket’ does not require my child to participate but I want my child/ward to do so, despite the risk and despite this waiver release.
I understand that there are foreseeable, unforeseeable and inherent risks of harm involved in the sport and activities of cricket. I acknowledge that the activities can be physically and mentally intense. I know and understand that the risk of injury from the activities are significant, including the potential for partial or total disability and death, and while personal discipline can minimize this risk, the risk for serious injury does always exist.
In exchange for the opportunity to participate in the practice/camps/training, I personally assume all risks in connection with my child’s participation, Additionally I, on behalf of my child, my spouse, and myself release ‘The Institute Of Cricket’ as well as its officers, employees, representatives, coaches/trainers, volunteers from any and all claims, judgements and liability from any injury including personal injury or death.
I, on behalf of my child/ward, my spouse, and myself, agree to indemnify, defend, and hold ‘The Institute Of Cricket’ harmless, as well as its officers, representatives, coaches/trainers, and volunteers from any and all liabilities, damages ,judgements and expenses that they may incur in connection with my child’s/ward participation.
I specifically assume all risks and hazards associated with my child’s participation in the camp/clinic/activity including, but not limited to, the risks associated with the COVID-19 or similar type virus. I understand that my child will be associating with Coach, volunteers, and other children and may contract COVID-19, and other viruses and diseases, through my child’s participation in the camp/clinic/activity. Although the children and staff/volunteers may have their temperatures taken upon entering the facility, that precaution is not nearly adequate to prevent the spread of COVID-19 given, among other things, the relatively long incubation period, and the fact that many infected persons are asymptomatic. I understand and voluntarily assume the risk that my child may acquire COVID-19, and that COVID-19 may subsequently be transmitted from my child to me, my family, and members of my household.
I certify that my child is in good health, has no fever, and has no current issues that make it unsafe for my child to participate in camp/clinic/activity, which may not have a medical professional on staff. I will notify 'The Institute Of Cricket' and not send my child to the camp/clinic/activity if my child develops a fever or illness or tests positive for COVID-19.
I also declare that my child/ward is participating in all the camps/practice with his/her own desire and willingness and ‘The Institute Of Cricket’ or its representatives have not coerced or influenced him/her in any way. In the eventuality of my child/ward or anyone living with him/her contracting Covid-19. The academy or its representatives, coaches/trainers, volunteers shall not be held liable legally and/or financially.
I also declare that during the tenure of this camp/program organized by ‘The Institute Of Cricket’ we will undertake to follow all the Government guidelines. Failing to do so will result in ‘The Institute Of Cricket’ having the discretion to take necessary action.
I voluntarily grant ‘The Institute Of Cricket’ the absolute and irrevocable right and unrestricted permission to use my name, likeness, image, voice and/or appearance in any photos, video recordings, audiotapes, digital images, and the like, taken or made on behalf of ‘The Institute Of Cricket’ or its parents. I agree that ‘The Institute Of Cricket’ has complete ownership of such material and can use said material for any purpose consistent with 'The Institute of Cricket' mission. These uses include, but are not limited to photographs, videos, publications, advertisements, news releases, websites and any promotional or educational materials in any medium.
